What a valid notice must include (17 U.S.C. §512(c)(3))

A physical or electronic signature of the copyright owner or a person authorized to act on their behalf.
Identification of the copyrighted work claimed to have been infringed (or a representative list, if multiple works).
Identification of the material claimed to be infringing, with information reasonably sufficient for us to locate it (a direct URL is best).
Your contact information: name, address, telephone number, and email address.
A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
A statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act on behalf of the copyright owner.

Incomplete notices may not be actionable. Knowingly material misrepresentations in a takedown notice can carry liability under 17 U.S.C. §512(f).

Counter-notices

If material you posted was removed and you believe the removal was a mistake or misidentification, you may send a counter-notice under 17 U.S.C. §512(g)(3) to the same address. A valid counter-notice has its own statutory requirements, including consent to federal court jurisdiction; we recommend reviewing them (or consulting counsel) before sending one.

Repeat infringers

We terminate the accounts or submission privileges of repeat infringers in appropriate circumstances.
